  • The main factors that cause a rise in the prevalence of fatty liver in India include changing lifestyles, unhealthy dietary choices, rising obesity rates, and increasing burden of metabolic disorders like diabetes and insulin resistance.

  • There are two kinds of fatty liver namely alcohol-linked fatty liver and non-alcoholic fatty liver. The major reason of alcohol-linked fatty liver remains the intake of higher alcohol.

  • Meanwhile, when it comes to non-alcoholic liver disease, the major factors remain obesity and sedentary lifestyle.

Fatty liver disease is emerging as a major health issue in India, impacting not only older people but increasingly youngers ones too. Fatty liver occurs when excess fat accumulates in the liver cells, harming the organ’s capacity to function effectively. While some fat in the liver is normal, excessive buildup can cause inflammation, liver damage, and in extreme cases even cirrhosis or liver failure.

Fatty liver can be categorised into non-alcoholic fatty live disease (NAFLD/MASLD) and alcoholic fatty liver diseases (AFLD). Metabolic dysfunction-associated steatotic liver disease (MASLD), earlier known by the name non-alcoholic fatty liver disease (NAFLD) is a liver condition that can impact individuals with higher weight, obesity, type 2 diabetes, high cholesterol, and high blood pressure.

The main factors that cause a rise in the prevalence of the disease in the country include changing lifestyles, unhealthy dietary choices, rising obesity rates, and increasing burden of metabolic disorders like diabetes and insulin resistance.

According to a study featured in National Library of Medicine, “Current concepts point to insulin resistance as the primary metabolic defect leading to NAFLD. Insulin resistance increases the influx of free fatty acids into the liver. This happens due to the failure of insulin to suppress the hormone-sensitive lipase, causing more FFA to be released from the adipose tissue. Also, elevated insulin levels and insulin resistance promote continuous synthesis of triglycerides in the liver.” 1

Types of Fatty Liver

There are two kinds of fatty liver namely alcohol-linked fatty liver and non-alcoholic fatty liver. The major reason of alcohol-linked fatty liver remains the intake of higher alcohol. Meanwhile, when it comes to non-alcoholic liver disease, the major factors remain obesity and sedentary lifestyle.

Recently, Ryan Fernando, renowned celebrity sports nutritionist and chief nutritionist at QUA Nutrition,, took to his Instagram handle and explained the 5 signs of fatty liver and nutrition tips to manage the disease.

Sharing the post the sports nutritionist said, “Fatty liver is rapidly becoming one of the most common metabolic conditions in India, even among people who don’t drink alcohol. Urban diets high in refined carbs, frequent eating out, poor protein quality, and long sitting hours are quietly driving this trend. What’s concerning is that it often develops without obvious symptoms in the early stages, making it easy to overlook until it progresses. But your body does drop subtle hints if you know where to look.” 2

Ryan said, India is seeing a sharp rise in NAFLD (fatty liver) due to the following reasons given below:

  • Frequent fried foods and seed oils

  • High refined carb intake (white rice, maida, sugar)

  • Low protein intake

  • Zero physical activity

  • Sedentary lifestyle

  • Excessive alcohol consumption

Signs Of Fatty Liver

1. Easy Bruising: The nutritionist said easy bruising is one of the most common signs of fatty liver. The liver makes essential clotting proteins like fibrinogen and prothrombin that help stop bleeding. When liver function is impaired, these factors are reduced, making clotting slower and causing easy bruising.

2. Itching (Especially At Night): Ryan said itching especially at night is also one of the signs of the disease. It is known as cholesterol pruritus (itching caused by a liver/bile problem). Decreased bile flow leads to buildup of bile salts under the skin that can lead to itching.

3. Fatigue & Low Energy: When the liver is overloaded with fat, it struggles to process nutrients and toxins efficiently. This can leave you feeling unusually tired, sluggish, or low energy, even after sufficient rest, added the celebrity nutritionist.

4. Fat Accumulation: It is common in Non-Alcoholic Fatty Liver Disease or NAFLD. It can be driven by insulin resistance, where excess glucose gets converted into fat and stored in the liver.

5. Brain Fog & Irritability: In advanced cases, toxins like ammonia are not cleared effectively that can affect brain function. It is also known as hepatic encephalopathy spectrum.

Nutrition Tips For Fatty Liver

  • Cut sugar and refined carbohydrates. Restrict the intake of soft drinks, sweets, and white bread.

  • Include adequate protein in the diet. It may include eggs, dals, paneer, and lean meat.

  • Add at least 8 to 10 fibre options in every meal. It may contain vegetables, fruits, and whole grains.

  • Select healthy fats as part of balanced diet. You may add nuts, seeds, olive oil, coconut oil, and fish.

  • Avoid fried and processed or packaged foods.

  • Limit or avoid alcohol.

FAQs

What are the key differences between alcoholic and non-alcoholic fatty liver disease?

Alcoholic fatty liver disease (AFLD) mainly results from excessive alcohol consumption, while non-alcoholic fatty liver disease (NAFLD or MASLD) is primarily linked to obesity, sedentary lifestyle, insulin resistance, and metabolic disorders like type 2 diabetes. NAFLD is increasingly common due to lifestyle changes and poor dietary habits.

How can dietary changes help manage fatty liver naturally?

Managing fatty liver naturally involves cutting sugar and refined carbs, avoiding fried and processed foods, and limiting alcohol intake. Incorporating adequate protein from eggs, dals, paneer, and lean meats along with 8 to 10 fiber-rich options like vegetables, fruits, and whole grains per meal supports liver health. Healthy fats from nuts, seeds, olive oil, coconut oil, and fish are also beneficial.

What are the most common early warning signs of fatty liver disease to watch for?

Early signs of fatty liver include easy bruising due to impaired clotting protein production, nocturnal itching from bile salt buildup, persistent fatigue despite rest, visible fat accumulation in the liver, and in advanced cases, brain fog and irritability caused by ineffective toxin clearance.

Is fatty liver reversible with lifestyle changes, and how quickly can improvements be seen?

Fatty liver, especially NAFLD, can often be reversed or its progression slowed with consistent lifestyle modifications including diet, physical activity, and weight management. The timeframe varies by individual, generally requiring months of sustained changes for significant liver health improvements.

How does insulin resistance contribute to the development of fatty liver disease?

Insulin resistance disrupts normal metabolic processes by increasing free fatty acid release from adipose tissue and promoting triglyceride synthesis in the liver. This causes excess fat to accumulate in liver cells, leading to inflammation and fatty liver disease, especially in people with obesity or type 2 diabetes.
